Vera Reception: Patient Release and Waiver

Plain English Summary: At Vera, we believe in radical transparency. Before you book, we need to be clear about our role. Vera is a technology platform that helps you research and connect with top-tier Providers. We are not a medical clinic, and we do not provide medical services. The actual aesthetic treatment is performed solely by the independent Provider you select. By booking through Vera Reception, you acknowledge that all medical decisions and risks are between you and that Provider.

In order to request an Appointment or book an Aesthetic Treatment through Vera Reception, you ("Patient" or "You") must accept this Patient Release and Waiver ("Waiver"). This Waiver is effective between Vera Beauty Inc. ("Vera," "we," or "us") and You as of the date when you first submit an Appointment Request. All capitalized terms not defined here have the meaning given to them in the Vera Terms of Service.

1. Vera is Not a Medical Provider

You acknowledge and agree that Vera is a technology platform that connects Patients with third-party Providers (clinics, doctors, and medical facilities).

No Medical Relationship: Vera is not a medical provider. We do not practice medicine, diagnose conditions, or perform treatments. No doctor-patient relationship is created between You and Vera.

Information Only: Any content on the Vera app, including results from our AIA (AI Aesthetician), facial analysis, Vera Research data, and the Vera Treatments List, is for informational and educational purposes only. It is not a substitute for professional medical advice, diagnosis, or treatment.

2. Assumption of Medical Risks

Participation in Aesthetic Treatments involves inherent risks that cannot be eliminated regardless of the care taken by the Provider. You understand that the Aesthetic Treatment(s) you book through Vera Reception (whether Surgical, Minimally Invasive, or Non-Invasive) carry the risk of:

• Physical injury, infection, scarring, or reaction to anesthesia.
• Dissatisfaction with the aesthetic outcome (subjective results).
• Complications requiring follow-up or corrective procedures.

TO THE MAXIMUM EXTENT PERMITTED UNDER APPLICABLE LAW, YOU KNOWINGLY AND FREELY ASSUME ALL RISKS, BOTH KNOWN AND UNKNOWN, ASSOCIATED WITH THE AESTHETIC TREATMENT(S) YOU RECEIVE FROM THE PROVIDER.

You acknowledge that the Provider – and not Vera – is solely responsible for assessing your suitability for any treatment, obtaining your informed medical consent, and delivering the treatment.

3. Travel and International Risks

If you are traveling to receive treatment (e.g., traveling to Korea for treatments), you acknowledge that international travel for medical procedures carries additional risks, including:

• Differing standards of care or medical regulations in the destination country.
• Language barriers during consultation (though Vera strives to assist, we cannot guarantee perfect translation).
• Travel disruptions, flight cancellations, or entry refusal that may cause you to miss your Appointment.
• Complications that may arise during your return travel post-treatment.

You assume full responsibility for your travel arrangements and any associated risks.

4. Release and Waiver

In consideration of being allowed to use Vera Reception to research and book your Appointment, You agree that:

• You have reasonably assessed the risks involved in the Aesthetic Treatment and have made an informed choice to proceed with the specific Provider.
• You alone are responsible for providing accurate medical history to the Provider and following their pre-and post-procedure instructions.

TO THE MAXIMUM EXTENT PERMITTED BY LAW, YOU RELEASE, HOLD HARMLESS, AND PROMISE NOT TO SUE VERA BEAUTY INC., ITS AFFILIATES, OFFICERS, EMPLOYEES, AND AGENTS (THE "RELEASEES") FOR ANY CLAIMS, DAMAGES, LOSSES, OR LIABILITIES OF ANY NATURE ARISING FROM OR IN CONNECTION WITH:

• THE QUALITY, SAFETY, OR OUTCOME OF THE AESTHETIC TREATMENT PROVIDED BY THE PROVIDER.
• ANY NEGLIGENCE, MALPRACTICE, OR MISCONDUCT BY THE PROVIDER.
• ANY ERRORS OR OMISSIONS IN THE INFORMATION PROVIDED BY VERA (INCLUDING AI RECOMMENDATIONS).

5. California Civil Code § 1542 Waiver

If you reside in California, you expressly waive the protection of Section 1542 of the California Civil Code, which provides: "A GENERAL RELEASE DOES NOT EXTEND TO CLAIMS WHICH THE CREDITOR DOES NOT KNOW OR SUSPECT TO EXIST IN HIS/HER FAVOR AT THE TIME OF EXECUTING THE RELEASE, WHICH IF KNOWN BY HIM/HER MUST HAVE MATERIALLY AFFECTED HIS/HER SETTLEMENT WITH THE DEBTOR."

6. Indemnification

You agree that if, despite this Waiver, You (or anyone on your behalf) make a claim against Vera relating to an injury or issue caused by a Provider, You will indemnify and hold Vera harmless from any liability, legal fees, or costs which Vera may incur as the result of such claim.

7. No Guarantee of Results

Vera strives to vet Providers for quality, but we cannot and do not guarantee specific results. Aesthetic medicine is an art as well as a science, and individual results vary. A lack of desired aesthetic result is not grounds for a claim against Vera.

BY CLICKING "AGREE & BOOK" OR SUBMITTING YOUR DEPOSIT, YOU AFFIRM THAT YOU HAVE READ THIS PATIENT RELEASE AND WAIVER, FULLY UNDERSTAND THAT YOU ARE GIVING UP SUBSTANTIAL RIGHTS (INCLUDING THE RIGHT TO SUE VERA FOR MEDICAL OUTCOMES), AND AGREE TO IT FREELY AND VOLUNTARILY.
